INSURANCE For insurance coverage the contractor must report 450 hours within 6 months. Hours worked within any given month are not reported until the 15th of the following month. Generally, you are eligible for benefits on the first day of the month following the month you meet the following initial eligibility requirements.-

            To maintain insurance after reaching eligibility 300 hours within a 3 month period must be reported or a total of 1200 per year. Health and Welfare hours are banked accumulated.

             Member Packets- the packets are mailed to the member as soon as he/she is eligible on the system with 450 hours. The packet includes an enrollment card to add dependents. In many cases the Fund Office does not have an address on file for the member, so please give your address to the local in which you belong or the fund office, or no packet can be mailed.

Insurance Coverage includes medical, optical and prescription coverage, medical Newforms, change of address and beneficiary cards can be obtained from our office. Upon eligibility you will recieive a prescription and vision cards. Prescription Dorchester SquareCo-pay is 20% and no deductible.
